We are sorry to hear that your child is leaving us. The purpose of this short questionnaire is to find out about your experience at Ohana - what was good and not so good, about our centre. All responses can be anonymous; it is up to you. It will take about 5 minutes to complete - thank you for taking the time to do this, we value your feedback in helping is making our care environment better.
